How to Report

Who is required to report?

Professionals required to report include the following:

  • Physicians, medical institutions, or nurses
  • School employees
  • Social workers
  • The inspector general appointed under § 43-4317

When is a report required?

When a mandated reporter has reasonable cause to believe that a child has been subjected to abuse or neglect, he or she shall report to the proper law enforcement agency or the Department of Health and Human Services on the toll-free number.

The report may be made orally by telephone with the caller giving his or her name and address and shall be followed by a written report.

What information should the report include?

Oral reports must include:

  • The name of the reporter
  • The reporter’s address

Written reports must include, to the extent available:

  • The address and age of the child
  • The address of the person(s) having custody of the child
  • The nature and extent of the abuse or neglect, or conditions and circumstances which would reasonably result in such child abuse or neglect
  • Any evidence of previous child abuse or neglect, including the nature and extent
  • Any other information that the reporter believes may be helpful in establishing:
    • The cause of the child abuse
    • The identity of the perpetrator(s)

Failure to report

Any person who willfully fails to make any report of child abuse or neglect required by § 28-711 shall be guilty of a class III misdemeanor.
